Home Telemonitoring Program for Heart Failure Treatment

American Outcomes Management (AOM) offers a comprehensive home telemonitoring program for managing your congestive heart failure treatment including inotropic therapies.

Whether you need early warning of decompensation in heart failure, improved treatment adherence, better use of nursing resources, reduced hospitalization, or better control of blood pressure, AOM's home telemonitoring program provides complete, reliable, cost-saving heart failure treatment solutions.

AOM's home telemonitoring program is an innovative service for congestive heart failure patients at risk for hospitalization who would benefit from daily evaluation by a registered nurse. The program includes daily remote vital sign monitoring and assessment, telephone intervention and customized education. Home telemonitoring supplements the home nursing visit schedule for patients.

Home Telemonitoring Heart Failure Treatment Services Offered:

• Vital sign screening (blood pressure/pulse)
• Weight monitoring
• Pulse oximetry
• Symptom monitoring

How Home Telemonitoring Works

Each day, the home telemonitoring system guides the patient through steps to collect vital signs or other physiological measurements specific to his or her needs. The data is transmitted to AOM where it is reviewed by a registered nurse and tracked for significant changes. The nurse responds with a telephone intervention that includes further assessment, teaching or communication with the patient's physician as needed.

Patients who have emergent medical/nursing issues can be assessed and advised over the phone. The patient care manager is notified of any problem(s) and any intervention so further follow-up can be provided if needed.

Home telemonitoring helps all members of the healthcare team, including the patient, to catch problems early before they turn into emergency needs. The goals are to decrease hospital admissions and help patients manage their healthcare at home.

In order to be eligible for our home telemonitoring program, you must:

• Be an active patient with AOM receiving heart failure treatment through home intravenous therapy
• Have a history of frequent hospitalizations, doctors' office or emergency room visits
• Experience frequent medication changes due to an unstable medical condition
• Require frequent monitoring of vital signs, medication adjustments, weight and blood oxygen
• Have no behavioral issues
• Be physically and cognitively able to follow the home telemonitoring monitor instructions or have a caregiver who can assist
• Have a residence that is safe and conducive to home telemonitoring
• Agree with the terms of participation
