Contract with AOM for Your Infusion Therapy Needs

American Outcomes Management has many active contracts with a variety of commercial payors, including traditional health plans, managed care organizations, preferred provider organizations, third-party administrators, self-insured plans, provider network managers, Medicare Part D plans, prescription drug plans and more. Contracting with AOM allows you to reduce spending without reducing benefits. You will benefit from significant cost savings and still deliver high quality services for your patients who require special care and medications outside of the hospital setting.

The AOM Referral Process - making your job easier

• AOM makes the referral process seamless by obtaining the necessary patient information from either the physician's office or the hospital discharge planner prior to contacting the payor for authorization.
• AOM also reviews all clinical information to ensure therapy is medically indicated prior to submitting to the payor for pre-certification/authorization.
• AOM acts as a liaison on behalf of the insurance case manager and physician to coordinate all medical documentation.

Refer Patients to AOM for Infusion Therapy

When insurance case managers refer eligible patients to AOM for infusion therapy, they become part of the AOM team. Placing a referral is fast and easy - simply fill out our online referral form and fax it to our referral line at 212-398-4859 or call a patient care manager at 800-746-9089. To refer a patient to AOM for infusion therapy, you will need to provide the following patient information:

• Name
• Address
• Phone number
• Social security number
• Date of birth
• Health insurance card(s) or name of insurance carrier
• Health insurance member ID number
• Diagnosis
• Allergies

If you can obtain past clinical notes or a letter of medical necessity and orders from the prescribing physician upfront, the referral process may be completed more quickly. Once the insurance case manager has submitted the referral form, we will verify the patient's benefits, obtain preauthorization if needed and set up the patient for home infusion therapy.
